With increasing experience, the role of interventional catheterization has become solidified as an integral component of the comprehensive management of patients with essentially all forms of congenital heart disease. Cardiac catheterization has also become established as a distinct subspecialty within the discipline of pediatric cardiology, and at many centers has become the purview of cardiologists who have received specialized training in interventional catheterization and whose practice is focused on catheterization. This book is targeted to trainees and established interventional cardiologists alike, and is intended as a comprehensive reference for the clinical practice of diagnostic and interventional cardiology in pediatric and congenital heart disease.In the organization of this book, a combination of procedure-based and problem-based approaches has been employed. The structure of the book has been updated to make it more reader friendly, chapters have been added to address new techniques and problems in the field, and greater emphasis has been placed on evaluation of outcomes of transcatheter interventions in congenital heart disease.Section 1 covers the fundamentals of cardiac catheterization, with chapters devoted to pre-procedure patient evaluation, sedation and anesthesia, vascular access and related issues, catheterization equipment ands its use, hemodynamic evaluation, angiography, radiation and radiation safety, and echocardiography in the catheterization lab.Sections 2 through 4 include procedure-based chapters that are organized according to the technical type of intervention. Section 2 is devoted to transcatheter interventions for obstructive lesions, including valvar stenoses and vascular obstructions. Section 3 includes chapters that cover interventions to close various pathologic intracardiac communications and vascular anomalies. Section 4 includes chapters covering interventions performed to create or enlarge intracardiac communications, as well as other interventions.Section 5 focuses on the applications of catheterization and transcatheter interventions in the evaluation and management of specific patient populations. This problem-based or patient-based focus, which is substantially expanded from the previous edition of the book, reflects the expanding role of catheterization in the comprehensive management of pediatric and congenital heart disease, and addresses emerging applications of transcatheter interventions, including management of the patient receiving extracorporeal circulatory support, hybrid transcatheter-surgical interventions, and prenatal cardiac intervention.Section 6 includes two chapters that cover outcome assessment and regulatory issues pertinent to the pediatric cardiology in general and.

As the applied radiation is not necessarily in the infrared, throughout the book we usually prefer the term optical transmission. As most equipment is battery-powered, the emphasis is on power optimiza tion of the optical transmission system. System parameters as well as environ mental parameters that determine the eventual transmission quality are iden tified, to facilitate well-reasoned system design. Many design rules, based on calculations, measurements and simulations are presented to help the designer push the performance close to the limits set by nature and the available tech nology. The first chapters introduce the subject and the present the scope of the book. Then, the basic transmission link is introduced in chapter 3, and strate gies to optimize its signal-to-noise ratio are discussed. MOTIVATION In many physical situations, a plant model is often provided with a qualitative or quantitative measure of associated model uncertainties. On the one hand, the validity of the model is guaranteed only inside a frequency band,… so that nearly nothing can be said about the behavior of the real plant at high frequencies. On the other hand, if the model is derived on the basis of physical equations, it can be parameterized as a function of a few physical parameters, which are usually not perfectly known in practice. This is e.g. the case in aeronautical systems: as an example, the ae- dynamic model of an airplane is derived from the flight mechanics eq- tions. When synthesizing the aircraft control law, it is then necessary to take into account uncertainties in the values of the stability derivatives, which correspond to the physical coefficients of the aerodynamic model. Moreover, this airplane model does not perfectly represent the be- vior of the real aircraft. As a simple example, the flight control system or the autopilot are usually synthesized just using the aerodynamic model, thus without accounting for the flexible mechanicalstructure: the c- responding dynamics are indeed considered as high frequency neglected 1 dynamics, with respect to the dynamics of the rigid model . 232 pp. Englisch.

MOTIVATION In many physical situations, a plant model is often provided with a qualitative or quantitative measure of associated model uncertainties. On the one hand, the validity of the model is guaranteed only inside a frequency band, so…that nearly nothing can be said about the behavior of the real plant at high frequencies. On the other hand, if the model is derived on the basis of physical equations, it can be parameterized as a function of a few physical parameters, which are usually not perfectly known in practice. This is e.g. the case in aeronautical systems: as an example, the ae- dynamic model of an airplane is derived from the flight mechanics eq- tions. When synthesizing the aircraft control law, it is then necessary to take into account uncertainties in the values of the stability derivatives, which correspond to the physical coefficients of the aerodynamic model. Moreover, this airplane model does not perfectly represent the be- vior of the real aircraft. The situation has now developed up to the point where finite element s…imulation software is sold commercially and routinely used in industry. The introduction of the finite element method in charged particle optics came by way of a PHD thesis written by Eric Munro at the University of Cambridge, England, in 1971 [1], shortly after the first papers appeared on its use to solve Electrical Engineering problems in the late sixties. Although many papers on the use of the finite element method in charged particle optics have been published since Munro's pioneering work, its development in this area has not as yet appeared in any textbook. This fact must be understood within a broader context. The first textbook on the finite element method in Electrical Engineering was published in 1983 [2]. At present, there are only a handful of other books that describe it in relation to Electrical Engineering topics [3], let alone charged particle optics. This is but a tiny fraction of the books dedicated to the finite element method in other subjects such as Civil Engineering. Interest has recently… increased in delta-sigma circuits built with a continuous-time loop filter rather than the more common switched-capacitor approach. Continuous-time delta-sigma modulators offer less noisy virtual ground nodes at the input, inherent protection against signal aliasing, and the potential to use a physical rather than an electrical integrator in the first stage for novel applications like accelerometers and magnetic flux sensors. More significantly, they relax settling time restrictions so that modulator clock rates can be raised. This opens the possibility of wideband (1 MHz or more) converters, possibly for use in radio applications at an intermediate frequency so that one or more stages of mixing might be done in the digital domain. Continuous-Time Delta-Sigma Modulators for High-Speed A/D Conversion: Theory, Practice and Fundamental Performance Limits covers all aspects of continuous-time delta-sigma modulator design, with particular emphasis on design for high clock speeds. The authors explain the ideal design of such modulators in terms of the well-understood discrete-time modulator design problem and provide design examples in Matlab. They also cover commonly-encountered non-idealities in continuous-time modulators and how they degrade performance, plus a wealth of material on the main problems (feedback path delays, clock jitter, and quantizer metastability) in very high-speed designs and how to avoid them. They also give a concrete design procedure for a real high-speed circuit which illustrates the tradeoffs in the selection of key parameters. Detailed circuit diagrams, simulation results and test results for an integrated continuous-time 4 GHz band-pass modulator for A/D conversion of 1 GHz analog signals are also presented.
