Edition originale de la thèse novatrice et remarquable de l'auteur, qui poursuivra ses travaux sur la phlébite utérine pendant plusieurs années. Jean Baptiste Hippolyte Dan…ce (1797-1832) fut interne des hôpitaux de Paris et fut reçu à l'agrégation de médecine au concours de 1826 ; il exerça à l'hôpital Cochin. Il décéda prématurément du choléra. Malgré la brieveté de sa carrière médicale, il eut une grande réputation. En plus de ces travaux pionniers et faisant date dans l'histoire de la bactériologie, il fut le premier à avoir individualisé la tétanie, sous le nom de tétanos intermittent (Loewenberg SA: Clinical Endocrinology, p. 343: "Dance, in 1831, published an accurate description of a case of tetany in which he mentioned the carpo-pedal spasm, the convulsions and the sharp piercing outcries of the patient."; Garrison & Morton, 4828). Avant la soutenance de cette thèse, Dance avait exercé pendant 2 ans à l'Hôtel-Dieu dans le service des femmes en couches. Il consacra sa thèse à cette maladie "dont l'étude semble avoir été négligée, quoiqu'elle soit assez fréquente". Il étudie successivement l'état de la femme après l'accouchement puis les caractères communs aux maladies des femmes nouvellement accouchées et le traitement le plus convenable. Il écrit : " toutes les maladies qui se déclarent après l'accouchement, connues sous le nom de maladie puerpérale, sont en général de nature inflammatoire", "toutes ces inflammations sont en général promptement purifères". Dance passe en revue quelques-unes des maladies qui lui semblent aggravées, sinon produites par cette "idiosynchrasie puerpérale" (péritonite puerpérale, abcès multiples, etc.) puis il présente une étude détaillée de la métrite puerpérale. Il affirme qu'elle est "bien souvent le point de départ de la péritonite", expose ses caractères anatomo-cliniques, au niveau de l'empreinte circulaire correspondant à l'insertion du placenta (dure, sèche, friable et brune) et avec présence de pus dans les veines utérines, voire dans l'épaisseur des parois de l'utérus. Parmi les facteurs prédisposants, Dance cite notamment les cas où "on est obligé d'introduire la main dans la cavité de la matrice", anticipant les constatations de SEMMELWEIS. Puis Dance rapporte 5 observations anatomo-cliniques. Doué du talent d'observation le plus remarquable, d'une extrême sagacité, d'une persévérance et d'une patience à toute épreuve ; soutenu par un jugement droit et lucide, par un esprit d'induction qui, dans les limites de la science expérimentale, avait toute l'étendue désirable ; sans prévention pour ou contre les doctrines régnantes ; possédant cette indépendance et cette bonne foi scientifique si rare, dans laquelle la vérité se soustrait aux meilleures intentions, Dance ne pouvait manquer d'être remarqué dans notre temps, où trop souvent les principes et les opinions ont été mis à la place des faits. On lui doit d'avoir éclairé un des points les plus importants de la pathologie, celui qui concerne l'inflammation des veines. Les conséquences de la phlébite, si habilement déduites par Dance, ont fourni des points de vue tout nouveaux sur l'altération du sang et sur divers états morbides mal conçus ou ridiculement expliqués avant lui. On trouve déjà dans sa thèse le germe des idées de l'auteur sur la phlébite utérine".
