Second, there is no on-time response for patients because patients are expected to wait until there turn to reach. Third, patient card's that holds patient history or profile might be lost during a follow-up visit, work overloads of health workers and so on. For this study, a cross-sectional study was conducted in different private and governmental hospitals, health centers and clinics. A purposive sampling technique is used to select domain experts for knowledge acquisition and different documents such as manual, guidelines, literature reviews, Internets consulted to acquire explicit knowledge. 124 pp. Englisch.
