MANAGEMENT OF ARRHYTHMIAS IN THE CORONARY CARE UNIT NORA GOLDSCHLAGER, RODERICK WOODS, AND NEAL BENOWITZ With the advent of coronary care units, mortality from cardiac arrhythmias occurring during acute myocardial infarction has been unquestionably and dramatically reduced. In addition, continuous electrocardiographic monitoring has resulted in the recognition of specific arrhythmias related to acute ischemic heart disease and thus to the development of appropriate strategies for their management. This chapter will review certain specific aspects of those bradyarrhythmias and atrial and ventricular tachyarrhythmias that are seen most commonly in acute myocardial infarction. Pharmacologic therapy, including the use of some of the newer antiarrhythmic agents, will be discussed, as will pacemaker ther apy of both bradyarrhythmias and tachyarrhythmias.

It is an attempt to establish a critical mass of oncology literature covering virtually all oncology topics, revised frequently to keep the coverage up to date, and easily available on a single library shelf or by a single personal subscription. We have approached the problem in the following fashion: first, by dividing the oncology literature into specific subdivisions such as lung cancer, geni tourinary cancer, pediatric oncology, etc.; and second, by asking eminent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. Each topic and tumor type is covered in a volume appearing frequently and predictably, discussing current diagnosis, staging, markers, all forms of treatment modalities,basic biology, and more.

It was felt that a short review of the main pathogenetic conceptions and therapeutic principles which have presented themselves with regard to mammary cancer in the course of Western history, might help to draw a more complete picture of where we stand today. It is not easy to decide which ideas, although discarded, deserve yet to be remembered and which authors from the past may be considered to be truly representative of the scientific climate of their age. Twenty centuries have produced quite a lot of ideas and the number of medical authors who advanced, or rejected, or modified, or revived them, is really uncountable. So the historian has to make a selec tion and choices are perforce subjective and open to criticism. In writing this book I tried to consult original sources in the original language as much as possible. These sources were not always strictly medical since I aimed at placing the problem of malignant breast disease - which might serve as a paradigm of cancer in general - in a somewhat wider context. The biologist has become interested in estuaries as areas in which to study the responses of animals and plants to severe environmental gradients. Gradients of salinity for example, and the problems of living in turbid water or a muddy substrate, prevent most animal species from the adjacent sea or rivers from entering estuaries. In spite of these problems, life in estuaries can be very abundant because estuarine mud is a rich food supply which can support a large number of animals with a large total weight and a high annual production. Indeed estuaries have been claimed to be among the most productive natural habitats in the world.
