The explanations advanced frequently hinge on those supply and demand factors, perceptible during the growth period itself, which may have helped or hindered economic progress. The problem which arises with this approach is whether those forces attributed with having pulled a country forward were the same as those which, in their absence, had held it back. For example, the growth of'inter national demand may be seen as a major stimulus in the economic development of a particular country, but its effectiveness as a stimu lant may have been contingent upon the prior removal of quite diffe rent obstacles to growth. In these circumstances it would be quite wrong to attribute lack of earlier development to the absence of international demand. Thus the study of a period preceeding discer nible growth in a sector of the economy may tell historians as much about the reasons for subsequent growth as a study of the growth period itself. After being introduced by Turgot in 1767 it has become accepted as one of the cornerstones of contemporary economic theory. My interest in this area started in the fall semester of 1971 at U.C. Berkeley where I was enrolled in Professor Ronald W. Shephard's class on the theory of production. Shephard introduced me to his work on the Law of Diminishing Returns, and encouraged me to continue that work. Abnormalities in the serum lipids are one of the major risk factors widely recognised as leading to this epidemic of heart disease. This is a very emotive area of medicine, one where often the patients most active in seeking advice have the least to gain. The individual doctor needs to decide for himself whether the cholesterol issue requires action or not, and for this he needs access to up to date and relevant data. This item is printed on demand - it takes 3-4 days longer - Neuware -The main aim of this study is to define the clinical criteria which must be considered in order to come to an adequate decision whether a patient with intracranial arteriovenous malformation (AVM) should be operated upon or treated conservatively. A special method was used to reduce the effects of selection. This method made it possible to evaluate the therapeutic efficacy of conservative treatment versus surgery. The method implies that patients with equal combinations of variables (risk profiles) were compared in the two treatment lines. The variables building up the risk-profile pattern were chosen by analysing the decision process, as it was originally practiced by the surgeon who selected and treated the patients of this study. The risk profiles thus described relevant characteristics of the patient and his malformation. The variables were assigned numerical values accord ing to their prognostic value. Summation of the variables making up the risk profile then gives each risk profile a certain value. A low value symbolizes a bad prognosis and a high value a good prognosis in both treatment groups. There were many risk profiles with the same value in both groups and a comparison could be made over a large part of the risk-profile scale. The risk profiles thus described relevant characteristics of the patient and his malformation. The variables were assigned numerical values accord ing to their prognostic value. Summation of the variables making up the risk profile then gives each risk profile a certain value. A low value symbolizes a bad prognosis and a high value a good prognosis in both treatment groups. There were many risk profiles with the same value in both groups and a comparison could be made over a large part of the risk-profile scale.
