"When the First World War came, Henry Head was appointed as consultant to the Empire Hospital in London for the reception of officers with neurological wounds. Assisted by Capt. George Riddoch, the Medical Officer in charge, he now immersed himself in the physiological problems brought about by transverse lesions of the spinal cord. The phenomenon which interested them most keenly consisted first in the so-called automatic bladder. This may come about even after the spinal cord has been completely divided. When the contents of the bladder reach a certain pressure-level, the wall contracts, the sphincter relaxes and urine is voided involuntarily. The second clinical features which interested Head and Riddoch entailed the excessive sweating, often paroxysmal in appearance, indicating an activity of the nervous system below the level of the lesion. Finally they studied certain reflex phenomena. Thus scratching the sole of the foot may give rise to a flexor spasm, premature evacuation of the bladder, and an outburst of excessive sweating" (Critchley, Black Hole & Other Essays, p. 102). "It is a commonplace that the spinal cord must be treated with the greatest respect at operation. Interference with it during removal of an extramedullary tumour, by injudicious or rough manipulation, is apt to leave the patient more paralysed than he was before operation, and this risk is greatly increased when attempts are made to remove a tumour from within the spinal cord itself. Removal of tumours from the cervical or upper thoracic spinal cord has at times been followed by respiratory failure or by fatal hyperthermia. It was doubtless for this reason that Elsberg and Beer introduced the "extrusion method" of treatment of intramedullary tumours. By this method at the first operation the tumour is exposed by an incision through the posterior columns, a week later the wound of operation is reopened and as much of the tumour as has meanwhile extruded itself through the incision in the spinal cord is removed. In this way the surgeon aims at removal of tumour tissue with minimum injury to the spinal cord itself. More recently it has been suggested that no attempt should be made to remove intramedullary tumours, but that instead they should be treated by X-rays, surgical measures being restricted to decompressive laminectomy. The object of this paper is to show that there are certain intramedullary gliomas that can be completely removed without serious permanent injury to the spinal cord." The role of George Riddoch has largely been ignored or mentioned merely in relation to Ludwig Guttmann and his appointment as the first Resident Medical Officer at Stoke Mandeville Hospital. Riddoch's contribution was far more significant. New material, comprising Riddoch's letters and memoranda written between 1939 and 1944, reveals his paramount involvement in the setting up of spinal injury units across the UK between 1941 and 1944, and his skill as an administrator and a clinician.
