Even if it seems desirable to include as many presentations of a conference as possible, the large number of excellent presentations at the VIIth World Symposium on Cardiac Pacing, 337 oral presentations and 236 posters, could not be published within an accepta ble time period. They are used worldwide with great success and a comparatively low incidence of adverse reactions. The most prevalent and threatening diseases in modem industrialized societies - the various forms and complications of coronary arteriosclerosis and arterial hypertension - are amenable to treatment with calcium antagonists. The pharmacological spectrum of calcium antagonists is highly specific and was supplemen ted a few years ago by gallopamil, a verapamil analogue. The experimental work reported at this symposium was concerned with effects of gallopamil on the electrophysiology of the heart, particularly in acute myocardial ischemia. New therapeutic features of myocardial hypertrophy and hypertrophic cardiomyopathy are discussed. In medical practice, gallopa mil has proved particularly useful for the treatment of angina pectoris. Studies comparing gallopamil with other calcium antagonists are described to profile the properties of the drugs in terms of their specific therapeutic value. There is still a need for more differential therapeutic trials. This first international symposium on gallopamil was attended by experts from institutions devoted to theoretical and clinical research and general practice. 204 pp. Englisch.

Regardless of the fact that many different and often unknown primary causes can result in heart failure, mechanical overload and myocardial hypertrophy is found in almost all forms of manifest chronic heart failure (apart from failure due to extramyocardial hindrances to inflow or to relaxation). However, the reactive enlargement of myocardial mass in response to an enhanced hemodynamic burden appears to be a double-edged sword. Obviously, the hypertrophy helps to reduce the enhanced ventricular wall stress in heart failure by adding contractile units to the overdistended chamber wall. However, in recent years it became clear that this adaptive hypertrophic process is rather complex and may include problematic facets. The adaptive hypertrophy includes proliferation of the nonmyocyte cardiac cells as well as substantial alterations in the phenotype of the growing myocytes due to differential changes in gene expression. 220 pp. Englisch.

Incidence and mortality of the disease are high, the patient is disabled, and is permanently threatened by the high morbidity and mortality. The clinician faces a syndrome of complex pathophysiology. Multiple causes or underlying disorders of the heart have to be differentiated from heart failure itself, which often results in exceedingly difficult diagnoses. Likewise, prognostication meets with difficulties due to problems in separating influences of the underlying disease and the heart failure syndrome itself. In chronic refractory failure annual mortality may exceed 50%. If aortic stenosis or ischemic cardiomyopathy with main stem lesions are present, this percentage may be even higher. The situation becomes particularly threatening to the patient when the reduction in cardiac performance goes along with complex ventricular arrhythmias. Therapy has remained difficult and of limited effectiveness. Major progress was achieved with the introduction of diuretic substances. Of similar importance was the introduction of va so dilating drugs into the treatment of heart failure. The principle of vasodilation has greatly improved our understanding of the disease, and has brought about a major improvement of symptoms, increase of exercise capacity, and reduc tion of mortality. This is especially true for the introduction of the angiotensin converting enzyme inhibitors. 372 pp. Englisch.

The proceedings of this symposium highlight the most re cent questions related to this topic. Apparently, partial tolerance, or rather partial decrease in sensitivity, can arise during nitrate therapy. Probably this is the consequence of a partial readjustment of the vascular system. This readjustment refers particularly to the arterial system, i.e., to the hypoten sive effect of nitrates in the standing position. A similar effect on the venous side is not present. On the other hand, the results assembled in this volume confirm the experience of many decades that long-term antianginal therapy with nitrates remains effective even when high doses are applied. Thus partial tolerance which also occurs after other vasodilators such as prazosin, does not imply tolerance against antianginal effectiveness. Soon after this on opposite sides ofthe world, Sir Brian Barratt-Boyes in Auckland, New Zealand, and Mr. Donald Ross in London, undertook to use allografts for the replacement of diseased aortic valves. Since that time the global interest in allografts has been patchy, episodic, and without a con sensus. Nonetheless, for the last 20 years at least three groups in the world have steadfastly pursued the development of new and relevant information concerning the use of allograft valves in humans. These are the centres of Sir Brian Barratt Boyes, Mr. Donald Ross, and Mark O'Brien in Brisbane. More recently, talented investigators, including Drs. Yankah, Yacoub, and others, have been developing information concerning the immunological aspects of the use of allografts, as well as their clinical use. No doubt, at present, cardiac valve allografts of one sort or another are the devices of choice for conduits and have an important place in the surgery of aortic valve replacement. Even so, in the mind of this writer at least, the future usefulness of allografts for the replacement of diseased cardiac valves and conduits between a ventricle and the pulmonary artery, remains problematic, and depends upon improvements in other devices for this purpose and upon improve ments that may be made in preparing and using allografts. 416 pp. Englisch.

Recent studies suggest that such benefit may also extend to patients with mild to moderate hypertension. The putative benefits of drugs may be offset, however, by their adverse effects and a prime example is the increased incidence of impotence and metabolic disorders in patients on thiazide diuretics. There is, therefore, a real need to look further into the therapy of patients with mild to moderate hypertension. The ideal drug in such patients would oppose the basic mechanisms responsible for the elevation in blood pressure, would prevent counter-regulatory responses and would have minimal side-effects in both the short-term and the long-term, the latter being of particular importance to younger patients. These aspects were considered at the symposium 'Mild hypertension. Current con troversies and new approaches' held at Titisee in West Germany, October 13-15 in 1983. The foundation for discussion was set with an exposition of the neural and hormonal regulation of blood pressure in normal man followed by a consideration of the pos~ible pathophysiological mechanisms involved in patients with hypertension. Particular at tention was focused on the central nervous system and on effects governed by activity of the peripheral nervous system as these may well provide further opportunities for logical therapeutic intervention in clinical hypertension. The role of ischemia in the genesis of ventricular arrhythmias and of sudden cardiac death has been observed beyond doubt. Regardless of the fact that many different and often unknown primary causes can result in heart failure, mechanical overload and myocardial hypertrophy is found in almost all forms of manifest chronic heart failure (apart from failure due to extramyocardial hindrances to inflow or to relaxation). However, the reactive enlargement of myocardial mass in response to an enhanced hemodynamic burden appears to be a double-edged sword. Obviously, the hypertrophy helps to reduce the enhanced ventricular wall stress in heart failure by adding contractile units to the overdistended chamber wall. However, in recent years it became clear that this adaptive hypertrophic process is rather complex and may include problematic facets. Neuware -There is clear evidence that in severe hypertension lowering blood pressure, by drug therapy decreases the incidence of major cardiovascular events. Recent studies suggest that such benefit may also extend to patients with mild to moderate hypertension. The putative benefits of drugs may be offset, however, by their adverse effects and a prime example is the increased incidence of impotence and metabolic disorders in patients on thiazide diuretics. There is, therefore, a real need to look further into the therapy of patients with mild to moderate hypertension. The ideal drug in such patients would oppose the basic mechanisms responsible for the elevation in blood pressure, would prevent counter-regulatory responses and would have minimal side-effects in both the short-term and the long-term, the latter being of particular importance to younger patients. These aspects were considered at the symposium 'Mild hypertension. Current con troversies and new approaches' held at Titisee in West Germany, October 13-15 in 1983. The foundation for discussion was set with an exposition of the neural and hormonal regulation of blood pressure in normal man followed by a consideration of the pos~ible pathophysiological mechanisms involved in patients with hypertension. Particular at tention was focused on the central nervous system and on effects governed by activity of the peripheral nervous system as these may well provide further opportunities for logical therapeutic intervention in clinical hypertension. Soon after this on opposite sides ofthe world, Sir Brian Barratt-Boyes in Auckland, New Zealand, and Mr. Donald Ross in London, undertook to use allografts for the replacement of diseased aortic valves. Since that time the global interest in allografts has been patchy, episodic, and without a con sensus. Nonetheless, for the last 20 years at least three groups in the world have steadfastly pursued the development of new and relevant information concerning the use of allograft valves in humans. These are the centres of Sir Brian Barratt Boyes, Mr. Donald Ross, and Mark O'Brien in Brisbane. More recently, talented investigators, including Drs. Yankah, Yacoub, and others, have been developing information concerning the immunological aspects of the use of allografts, as well as their clinical use. No doubt, at present, cardiac valve allografts of one sort or another are the devices of choice for conduits and have an important place in the surgery of aortic valve replacement. Incidence and mortality of the disease are high, the patient is disabled, and is permanently threatened by the high morbidity and mortality. The clinician faces a syndrome of complex pathophysiology. Multiple causes or underlying disorders of the heart have to be differentiated from heart failure itself, which often results in exceedingly difficult diagnoses. Likewise, prognostication meets with difficulties due to problems in separating influences of the underlying disease and the heart failure syndrome itself. In chronic refractory failure annual mortality may exceed 50%. If aortic stenosis or ischemic cardiomyopathy with main stem lesions are present, this percentage may be even higher. The situation becomes particularly threatening to the patient when the reduction in cardiac performance goes along with complex ventricular arrhythmias. Therapy has remained difficult and of limited effectiveness. Major progress was achieved with the introduction of diuretic substances. Of similar importance was the introduction of va so dilating drugs into the treatment of heart failure. The principle of vasodilation has greatly improved our understanding of the disease, and has brought about a major improvement of symptoms, increase of exercise capacity, and reduc tion of mortality. They are used worldwide with great success and a comparatively low incidence of adverse reactions. The most prevalent and threatening diseases in modem industrialized societies - the various forms and complications of coronary arteriosclerosis and arterial hypertension - are amenable to treatment with calcium antagonists. The pharmacological spectrum of calcium antagonists is highly specific and was supplemen ted a few years ago by gallopamil, a verapamil analogue. The experimental work reported at this symposium was concerned with effects of gallopamil on the electrophysiology of the heart, particularly in acute myocardial ischemia. New therapeutic features of myocardial hypertrophy and hypertrophic cardiomyopathy are discussed. In medical practice, gallopa mil has proved particularly useful for the treatment of angina pectoris. Studies comparing gallopamil with other calcium antagonists are described to profile the properties of the drugs in terms of their specific therapeutic value. There is still a need for more differential therapeutic trials. Identifikation von Risikopatienten und adäquate effektive antiarrhythmische Therapie sind daher von gröBter Bedeutung. Systematische Studien zeigen, daS mit den derzeit verfügbaren Antiarrhythmika nur ca. 50-60% der Patienten mit lebensbedrohlichen ventrikulären Tachykardien bei strenger Thera piekontrolle effektiv eingestellt werden können. Dabei sind in der praktischen Therapie nur wenige Antiarrhythmika verfügbar, die mit einer ausreichend hohen Responderrate einsetzbar sind. Andererseits stehen nichtpharmakologische BehandlungsmaSnahmen nur in begrenztem Umfang zur Verfügung, der klinische Stellenwert dieser Verfahren ist teilweise (z. B. Katheter ablation) noch nicht definiert. Es besteht somit ein Bedarf mr neue Antiarrhythmika, die sich in der Therapie lebensbedro hender Rhythmusstörungen als effektiv erweisen. Diprafenon, chemisch ein Propafenon Analogon, ist ein neues Antiarrhythmikum der Klasse le, we1ches über ausgeprägte betasympa tholytische Eigenschaften verfügt. Der vorliegende Band enthält eine erste Zusammenstellung der bisherigen experimentellen, pharmakokinetischen, pharmakodynarnischen und klinischen Befunde, die anläBlich eines Symposiums 1987 in Berlin zusammengetragen wurden. In den vergangenen Jahren sind sowohl bei der Entwicklung und dem Einsatz diagnostischer Verfahren als auch bei der medikamentosen, invasiven und chirurgischen Therapie entscheidende Fortschritte erzielt worden. Eine Vielzahl von Substanzen steht zur Pharmakotherapie der koronaren Herzkrankheit zur Verfu gung, inbesondere Nitrate, B-Blocker und Kalziumantagonisten, die einzeln oder in Kombination eingesetzt werden. Auf der anderen Seite hat die experimentell-kar diologische Forschung zunehmend das dynamische und komplexe Szenario der ischamischen Herzkrankheit betont, das Endothelschadigung, Gerinnungsprozesse, Veranderungen des Herzstoffwechsels und der Struktur, koronarkonstriktive Mechanismen, Umverteilungen der Durchblutung, Arrhythmogenese, kontraktile Dysfunktion wahrend Ischamie und Reperfusion und schlieBlich Vorhandensein oder Fehlen einer Schmerzempfindung umfaBt. Es erscheint daher unbedingt wiin schenswert, die Lucke zwischen experimenteller und klinischer Kardiologie zu schlieBen und damit eine pathophysiologische Grundlage fur rational begrundete diagnostische und therapeutische Entscheidungen zu liefern.
