Casually browsing through journals and conference proceedings to remain aware of developments in areas outside our specialization has become an even greater challenge. Frontiers of Computing Systems Research has been established to ad dress these two issues. With the assistance of an advisory board of experts from a wide variety of specialized areas, we hope to provide roughly annual volumes of invited chapters on a broad range of topics and designed for an interdisciplinary research audience. No single volume can cover all the rel evant topics and no single article can convey the full set of directions being pursued within a given topic. For this reason, a chapter listing technical reports available from universities is also included. Often, such unpub lished reports are designed for a general research audience and provide a good, informal look at trends in specialized research topics.

Significant advances were made during World War II, when the potential military applications of being able 'to see in the dar~' were demonstrated, and this progress has been maintained during the last forty years when many major advances have been achieved, such that the use of photodetectors for both civil and military applications is now relatively common and can be inexpensive.

In contrast, the art of tools for designing and producing analog or even analogi digital integrated circuits is quite primitive and still lack ing the industrial penetration and acceptance already achieved by digital counterparts. In fact, analog design is commonly perceived to be one of the most knowledge-intensive design tasks and analog circuits are still designed, largely by hand, by expert intimately familiar with nuances of the target application and integrated circuit fabrication process. The techniques needed to build good analog circuits seem to exist solely as expertise invested in individual designers.

Since that auspici ous event, nine international symposia on the role of taurine in biology have taken place. The locations for these meetings have been Tucson (tw…o times), Rome, Philadelphia, Tokyo, Vancouver, Mexico City, Helsinki, and Florence. In 1977, due to the large number of scientists in Japan who were interested in the role of this unique amino acid in biological systems, we organized the Japanese Research Society on Sulfur Amino Acids with the encouragement and financial assistance of the Taisho Pharmaceutical Co., Ltd (Tokyo). Annual meetings have been held, and the membership has expanded from 78 to 414 in 1987; the number of presentations has increased during this time span from 29 to 74. The symposium in Tokyo in 1982, 'Sulfur Amino Acids, Biochemical and Clinical Aspects' [1], was held to celebrate the 5th Annual Meeting of our Society. I would like to emphasize that in Japan we have an active Research Society especially directed to the study of sulfur amino acids. We have published our own semi-annual journal entitled Sulfur Amino Acids. Our society is an inter disciplinary research society since taurine is a highly diversified compound that interconnects physiology, biochemistry, pharmacology, nutrition, and medicine. One exciting fringe benefit of taurine research and the society has been the fostering of contacts with distinguished scientists from many varied medical fields.

Despite these screening efforts, carcinoma of the cervix rema…ins one of the more common malignancies in women and it is the leading cause of cancer death in many countries in the western hemisphere. It is estimated that in 1986 there will still be 14,000 new cases of invasive cancer, with 6,800 deaths in the United States alone. Unfortunately, many of these patients present with advanced disease, posing difficult management problems for the clinician responsible for their care. The treatment of early stage invasive carcinoma of the cervix (lesions confined to the cervix and vagina) remains either radical surgery, radical radiation therapy or a combination thereof This approach is extraordinarily effective in the vast majority of patients. However, there remains a subset of patients with early stage disease that are at high risk for recurrence. Dr Kjorstad (Chapter 2) has identified adenocarcinomas and adenosqua mous carcinomas as having a particularly poor prognosis. In addition, patients with more than three positive lymph nodes or with involvement of lymph nodes outside of the pelvis have a very poor prognosis. He has iden tified the CEA as a potentially predictive marker for these patients with poor prognosis, especially in patients with adenocarcinomas.

Put simply, we noticed that many VLSI design tools looked 'alike'. That is, at least at the overall software architecture level, the algorithms and data structures required to solve proble…m X looked much like those required to solve problem X'. Unfortunately, this resemblance is often of little help in actually writing the software for problem X' given the software for problem X. In the VLSI CAD world, technology changes rapidly enough that design software must continually strive to keep up. And of course, VLSI design software, and engineering design software in general, is often exquisitely sensitive to some aspects of the domain (technology) in which it operates. Modest changes in functionality have an unfortunate tendency to require substantial (and time-consuming) internal software modifications. Now, observing that large engineering software systems are technology dependent is not particularly clever. However, we believe that our approach to xiv Preface dealing with this problem took an interesting new direction. We chose to investigate the extent to which automatic programming ideas cold be used to synthesize such software systems from high-level specifications. This book is one of the results of that effort.

Since sodium is also considered to influence the cardiac contractile force by chang 2 ing the intracellular concentration of calcium through the Na +-Ca+ exchange 2 mechanism in the cell membrane, the role of Na +-Ca+ exchange in heart func tion as well as pathology of contractile failure is discussed. In view of the new ly discovered implications of the oxygen free radicals in cellular injury, papers concerning the role of these radicals in heart disease are included in this book. In time, experience led all of us from the several disciplines involved in this enterprise to the conclusion that the crude diag nostic techniques available to us circa 1945-1965 had garnered us little valid information upon which to base such complex, differential diagnostic decisions. It now is gratifying to look back and review the remarkable progress that has occurred in the field of clinical neuropsychology in the four decades since I was a graduate student. In the late 1940s such pioneers as Ward Halstead, Alexander Luria, George Yacorzynski, Hans-Lukas Teuber, and Arthur Benton already were involved in clinical studies that, by the late 1960s, would markedly have improved the quality of clinical practice. However, the only psychological tests that the clinical psychologist of my immediate post-Second World War generation had as aids for the diagnosis of neurologically based conditions involving cognitive deficit were such old standbys as the Wechsler Bellevue, Rorschach, Draw A Person, Bender Gestalt, and Graham Kendall Memory for Designs Test.

Even in the 'Scuola Salernitana', the oldest medieval medical school in Western Europe, about 1000 years ago it was taught how to improve urine output. The list of known 'diuretica' included herbs, plants, roots, vege…tables, in particular asparagus, fennel and carrot. The first diuretic drugs, however, were mercurial compounds. Thus, calomel, mercurous chloride, was initially used as a diuretic in the sixteenth century by Paracelsus, being one of the ingredients of the so-called 'Guy's Hospital pill'. But calomel had a cathartic effect so that it was replaced by organic mercurial compounds. These diuretics were clearly toxic. After the discovery of the car bonic anhydrase, in the early 1930s, and the introduction of sulfanilamide as a chemotherapeutic agent, it was observed that this drug was inhibiting carbonic anhydrase in vitro and urinary acidification in vivo thereby causing metabolic acidosis; urine output, however, appeared to increase. Subsequent studies led to the synthesis of more potent analogs, in particular acetazolamide. Studies on car bonic anhydrase inhibitors led to the synthesis of benzothiadiazides which disclosed much less inactivating action on carbonic anhydrase and much more diuretic effect through an inhibition of tubular transport of sodium and chloride. Chlorothiazide was the first member of this class of diuretics. Thiazides are still used in clinical practice.

Unfortunately, Dr. Krijnen suddenly died on the first of June 1989. In honour and mem ory of Dr. Krijnen this symposium will therefore be dedicated to him. Since the lOth International Symposium on Blood Transfusion in 1985 highlighted the theme of 'Future developments in blood banking', major changes have occurred in the blood banking world. Most of these changes were forced upon the Blood Banks by the fear of spreading AIDS through contaminated donations. This not only led to the wide spread testing of blood, but also to a more appropriate counselling of the community and the blood donors in specific. Additionally, virus inacti vation techniques were introduced for those components derived from multiple donations and intended for a regular transfusion in haemophi lia patients and others.

Congestive heart failure also is one of the commonest causes for hospital admissions accounting for over 5 million hospital days per year. Despite the early reco…gnition of this condition and active medical research into both mechanisms and therapy, prognosis continues to remain dismal wi th less than a 50% expected five year survival. In the last decade we have seen many new medical and therapeutic options for patients with congestive heart failure which extend beyond the use of bed rest, sodium restriction, digitalis and diuretics. These include vasodilators of a variety of types including the angiotensin conventional enzyme (ACE) inhibitors. Also, many new inotropes are under active investigation both in oral and intravenous forms. In March of 1984 a survey of over 5000 physicians was performed under the auspices of the American Heart Association (reported in: JAOC 8:966, 1986). That survey showed that there was no universally accepted defini tion for congestive heart fai lure and that a wide spectrum of diagnostic cri teria for this common condi tion existed even among academic cardiologists. There was no clear standard as to even the mos t bas ic treatment of conges t i ve heart fai lure. For example, exercise restriction was recommended by 19% of physicians, 31% recommended no change in activity, and 50% either light exercise or an exercise conditioning program.

Sir Richard Bright in 1827 associated pro with the disease t…hat bears his name. In the subsequent more teinuria than a century and a half, however, the meaning of the linkage between proteinuria and renal disease remains elusive. Proteinuria is discovered on routine urinalysis in about 10 million Americans, most of whom express no symptoms of kidney disease, each year. From the studies of Robinson (updated in these pages), we know that proteinuria, per se, can be present for 20 years without change in re nal function, as described in orthostatic proteinuria. By contrast, pro teinuria may be the harbinger of swift kidney destruction, rarely cul minating in clinical collapse, a syndrome typifying 'malignant proteinuria' as detailed herein by Avram. Although proteinuria is ubiquitous, an orderly management strategy for rational handling of proteinuria of less than nephrotic range is lack ing. Separation of tubular proteinuria and transient proteinuria of fever is now possible routinely. By that time it is esti mated that almost 20% of the population in most developed countries will be over the age of 65 and at a conservative estimate 1 in 10 of them will suffer from a dementing illness. Many symposia have appeared over the last few years describing the neuropathological and neurochemical deficiencies in Alzheimer's and other forms of dementia, the neuropsychological features of the disease, and attempts to treat it. In this volume, we have selected topics and authors who are beginning to question some of the earlier assumptions and to ask different questions about dementia. In the first four chapters the neuropathology and neu rochemistry of dementia are reevaluated. It is important to understand the relationship between the formation of amyloid plaques and neurofibrillary tangles, the progressive degeneration in cortex, and the neurochemical deafferentation of cortex. The first one was Hayashibara Forum '92: International Symposium on New Bases for Engineering Science, Algorithms, Dynamics and Fracta…ls held at Fujisaki Institute of Hayashibara Biochemical Laboratories, Inc. in Okayama during November 23-28 in which 49 mathematicians including 19 from abroad participated. They include both pure and applied mathematicians of diversified backgrounds and represented 11 coun tries. The organizing committee consisted of the following domestic members and Mike KEANE from Delft: Masayosi HATA, Shunji ITO, Yuji ITO, Teturo KAMAE (chairman), Hitoshi NAKADA, Satoshi TAKAHASHI, Yoichiro TAKAHASHI, Masaya YAMAGUTI The second one was held at the Research Institute for Mathematical Science at Kyoto University from November 30 to December 2 with emphasis on pure mathematical side in which more than 80 mathematicians participated.
