A new chapter on man's use of the oceans has been included to cover satellites and position fixing, renewable energy sources in the sea, seabed minerals, oil and gas, pollution and maritime law. In this edition we have also referred to a number of original references and review articles so that readers can find their way into the literature more easily. As in the first edition, PSM has been mainly responsible for the text and lIC for the illustrations, although each has responded to advice from the other and also from many colleagues. The text will almost certainly be too concise for many readers if they do not study the illustrations carefully at the same time. The alarming incidence figures of abuse and neglect directed toward family members justify this atten tion. For example, over 1 million children are thought to be abused and neglected each year. Similarly, almost 2 million women are victims of wife battering each year. Annual rates of elderly mistreatment are thought to be as high as 32 per 1000 population. Accurate epidemiologi cal data only now are being compiled on more recently recognized forms of mistreatment, such as psychological abuse, ritualistic abuse of chil dren, and child witnessing of adult violence. The pervasiveness of do mestic mistreatment makes it a priority for clinicians and researchers alike. For clinicians, intrafamilial violence represents a formidable chal lenge with respect to assessment and treatment. The etiology of abuse and neglect is multidetermined. There are numerous pathways in the development of family violence, and these interact and converge in a nonlinear fashion. The consequences of family violence are equally com plex and divergent. Victims of mistreatment can display a variety of physical injuries and psychological disturbances. No single psychiatric syndrome or symptom constellation has been consistently implicated in any form of family mistreatment. The perpetrators of family violence are equally heterogeneous in their clinical presentations. NHS nursing homes have been created, enthusiasts have developed new initiatives, and attempts have been made to improve attitudes. The recent White Paper on Community Care is likely to accelerate the transfer of large numbers of elderly patients from long-stay hospital beds to private-sector accommodation: a move often accompanied by much anxiety about standards of care. Although Health Authorities visit private nursing homes and apply the National Association of Health Authorities' standards to them, anxieties continue. Unfortunately for the patients who remain in hospital, many Health Authorities seem reluctant to apply these same standards to their own long-stay departments, since many would fail abysmally. The 1987 annual report of the Health Advisory Service (see Chapter 3) presents a damning indictment of the care given to old people: A recent review of twelve consecutive HAS Reports on services for older people in hospitals shows that long-stay wards consistently offered environments which were unable to offer privacy, homely surroundings, personal space and possessions or adequate furniture. In the twelve districts there was not one comprehensive personalised clothing service. Half of the reports commented on the lack of effec tive management of continence. The past. the present . . . and the future It is possible to take the view that ever since it began, the 'ancient' branch of physics known as Optics has been concerned with process ing images. But since the Nineteen-Thirties increasingly close ties have been forming between Optics, which until then had been largely based on instruments, and the sciences of communication and infor mation arising out of mathematics and electronics. Such developments follow naturally, since communication systems and image-forming systems are all designed to receive or transmit information. Further more the same mathematical forms are used for describing the beha viour of electrical and optical systems. It is a question of systems theory, particularly linear systems, and of Fourier's analysis methods, which together constitute an important part of Signal Theory. In the case of communication systems carrying signals of an electrical nature, information is time-related or temporal. Transmitted signals are one-dimensional and functions of a single variable, time t. In the case of optical systems information is spatial in nature. Signals are distributions of light intensity in space. In general they are treated as two-dimensional signals, being functions of two spatia! variables written as x and y. I was giving the third project tutorial of the afternoon. The first had been to a BA (Business Studies) student, the second to a part-time MBA student, and the third to a student registered on the Diploma in Personnel Management programme, and a great variety of issues had been dealt with during the course of the time involved. Nevertheless, I noticed that some of the material was common to all three students. I found myself thinking that I was repeating myself, and wanting to get through the basics as quickly as possible so that we could move on to the specifics of each particular project, which we both, each student and I, found more interesting to deal with. Unfortunately, the basics were precisely those topics which I considered essential to the success of any project. What's more, they dealt with the sort of material which wouldn't, on the whole, have occurred spontaneously to many students, and so it was a necessary part of my job to go through them. It was then seen as a contribution towards improved management and policy-making in a diffuse and neglected area of NHS management. The focus of the book is the 'old' Community Health Services: those transferred to Area Health Authorities from local authorities in the 1974 re organization of the NHS. These diverse services, while grouped together, had little objectively in common, occupying, as they do, a hazy middle ground between hospital and Family Practitioner Services. However, since 1974 there have been a number of major devel opments which have opened opportunities for change and devel opment in these services. These include: the resurrection of concern with 'Public Health'; the attempted closure of large mental illness and mental handicap hospitals and the development of 'Com munity Care'; the introduction of General Management; and the implication for health and local authorities of the White Papers 'Caring for People', 'Promoting Better Health' and 'Working for Patients'. Traditionally, Community Health Services were seen as low status and a professional dead-end. This, in tum, has led to a rather uneven body of literature. The growth of general management has led to a demand for a more coherent, management-orientated literature. Although the literature contains references to the survival of one baby with oesophageal atresia (without fistula) who was born in 1935, the major component of the oesophageal atresia story concerns the most frequent anomaly, namely oesophageal atresia with a distal tracheo-oesophageal fistula. The first long-term survivals of babies born with this anomaly were in 1939; it is appropriate therefore that this book should be compiled 50 years later. Surgery and neonatal care have made striking advances during this half century, and nowhere is this more obvious than in the field of neonatal surgery. But the care of the baby with oesophageal atresia requires more than a surgeon and a neonatologist, and our experience has shown the need for a multidisci plinary approach involving anaesthetists, intensive care therapists, thor acic physicians, general paediatricians, cardiologists and cardiac surgeons, orthopaedic surgeons, radiologists, nephrologists and geneti cists. Despite the advances in surgical treatment of this disease, particularly by reconstruction, it is a sad fact that the number of amputations performed in these countries each year for vascular disease is increasing.
