Although we introduced the term 'adult respiratory distress syndrome' in our second and more detailed description of the syndrome (ehest, 1971), this was… probably amistake for the simple reason that children also suffer the same syndrome fo11owing acute lung insults. Today, the syndrome of acute respiratory distress in adults (ARDS) is recognized as a worldwide problem, but the prevalence of disease varies in different parts of the world. A huge amount of research has focused on the mechanisms of acute lung injury and yet the exact sequence of events and media tors in inflammatory cascade, which result in acute respiratory failure from ARDS, is not known but many possibilities exist. The definition of ARDS has been gradua11y modified in recent years and investigators around the world are now co11aborating in order to establish more uniform concepts in identification, risk factors and mechanisms of lung injury, which someday will result in improved approaches to management. Already, at least some centers are showing improved outcomes in ARDS, achieving an approximate 60% survival rate. In the past, most large series documented only about a 40% survivability taking a11 causes of ARDS. This apparent progress is likely attributable to more meticulous and disciplined care than any specific pharmacologic attack on the basic mechanism resulting in ARDS.

This imposes the need not to encumber readers with the customary glut of academic references in the text, and to refer only to the best known and politically most influential theories and to authors who are also widely known to people who are not professional economists.

The workshop is a meeting place where both research and industry, theory and practice come together. By bringing both researchers and practitioners together, IWPTS opens up the communication between these groups. This helps keep the research vital and improves the state of the practitioner's art. Forty-eight papers have been submitted to IWPTS'95 and all of them have been reviewed by the members of the Program Committee and additional reviewers. The completed reviewers list is included in this Proceedings. Based on these reviews, the Program Committee selected 26 for oral presentation and 4 to be presented as posters. One thing is certain, however - the move towards interdiscipli nary working is here to stay. Not all change has been led by legislation, and many innovations have been founded in the day-to-day practices in the care of older people with mental health problems. A service, of course, does not become integrated merely by imposing joint working on a number of professionally based disciplines, and in many ways this may not be desirable. At its worst it produces duplication, where people from different background are all doing the same job. Since Newton's time it had been assumed that the problem was solvable. All that was needed was a little ingenuity and considerable perseverance, but Poincare saw that this was not the case. Strange, unexplainable things happened when he delved into the problem; it was not solvable after all. Poincare was shocked and dismayed by the result-so disheartened he left the problem and went on to other things. What Poincare was seeing was the first glimpse of a phe nomenon we now call chaos. With his discovery the area lay dormant for almost 90 years. Not a single book was written about the phenomenon, and only a trickle of papers appeared. Then, about 1980 a resurgence of interest began, and thousands of papers appeared along with dozens of books.
