L'histoire de l'assistance aux « insensés » à Aix-en-Provence s'insère dans le mouvement qui avait débuté en France après la Fronde et qui s'est développé d'abord sous direction religieuse (action de saint Vincent de Paul, fondation de l'ordre de Saint Jean-de-Dieu). Puis dès la fin du XVI siècle, apparaissent les établissements charitables ou dépendant des pouvoirs publics. Les développements qui permettaient l'institution de l'Hôpital Général (édit de 1656 et arrêt Pogel du Parlement de Paris en 1660) n'y sont pas étrangers. Mais en Provence, la première mesure officielle l'a été hors du royaume, en Avignon, par une bulle de 1681, permettant l'érection d'un établissement pour les insensés sous le rocher des Doms , dans une ancienne prison. Aix et Marseille ont fondé leurs maisons à peu près simultanément. L'un de nous a narré ici même , en 1975, l'histoire de la « Maison des Fous de Marseille ». L'Hôtel-Dieu d'Aix ou Hôpital Saint-Jacques, fondé en 1531, admettait les insensés calmes temporairement mais refusait en règle les agités, comme n'étant pas de son ressort. Ces furieux furent admis à l'Hôpital de la Charité, lorsqu'il ouvrit en 1640, et simplement ségrégés dans les locaux réservés aux mendiants, aux vieillards et aux orphelins. Soit en raison de l'augmentation de leur nombre, soit surtout du fait que « leur présence n'était plus tolérable », une réunion commune des recteurs des trois hôpitaux d'Aix (Saint-Jacques, la Charité et la Miséricorde) se tint le 2 septembre 1691 pour approuver la création d'un hôpital pour insensés. Ainsi débuta cette oeuvre hospitalière due à la charité privée et grâce aux ressources fournies par les hôpitaux, les consuls n'ayant envisagé que la possibilité de prélever, sur le produit des amendes de police, une subvention de 300 livres par an. Les trois recteurs de chacun des hôpitaux partagèrent la direction de l'oeuvre. Le règlement homologué par arrêt de la Cour du Parlement au 10 février 1695, comportait diverses particularités intéressantes. On notera que le point de vue médical proprement dit est laissé à l'appréciation des recteurs, et que l'administration comme la surveillance des pensionnaires est civile, le point de vue religieux n'étant pas négligé, mais sans l'intervention d'une autorité ecclésiastique dans le fonctionnement de la maison.
