Ihre Strecken sind hauptsächlich dampfbetrieben und gelten als eine überregionale Touristenattraktion. Das meterspurige Streckennetz erstreckt sich über eine Länge von 140,4?km und ist ganzjährig in Betrieb. Deshalb nennt sich die HSB "Die Größte unter den Kleinen". Das Netz besteht aus den drei Einzelstrecken Harzquer-, Selketal- und Brockenbahn. Die Anfangsbuchstaben dieser Strecken ergeben HSB, was dem abgekürzten Namen des Unternehmens entspricht. Drei der sechs Endbahnhöfe (Nordhausen Nord, Quedlinburg und Wernigerode) verfügen über einen Anschluss an das regelspurige Netz der Deutschen Bahn. Im Personenverkehr werden die Strecken heute überwiegend von Touristen, Ausflüglern und Wanderern genutzt. Der eigentliche Regionalverkehr im Sinne von Berufs- und Schülerverkehr spielt nur noch im Raum Nordhausen-Ilfeld eine bedeutende Rolle.
