Condición: Gut. 144 S.; 8°. Geheftet. Gutes Ex.; Einband etwas berieben; geringe Gebrauchsspuren. - INHALT : Editor's Notes -- The D--State: A Review and Discussion of Studies on the Physiologic State Concomitant with Dreaming Ernest L Hartmann (U S A ) The term "D--state" refers to the physiological state, marked by rapid eye m…ovements and low--voltage fast EEGs, that accompanies dreaming This state resembles ordinary sleep as little as it resembles waking Knowledge in this field is growing in many directions like a young octopus This article attempts to indicate some of the directions of growth and what the tentacles seem to be grasping -- Discussions: -- M Jouvet (France) -- An excellent review of this new field of research As a neuro--physiologist I should like to emphasize some of the fascinating unsolved problems: Why aren't the phylogeny and ontogeny of the D--state similar? Why does it vary so much among mammalian species? Why is it so closely correlated with sexual life in rabbits? -- Tomas Weiss (Czechoslovakia) -- \ There is almost complete lack of knowledge concerning the function of the rhombencephalic sleep phase Hartmann is wrong in regarding the question of function as not a very useful one The term "D--state" lacks definite content and meaning Any new term should be based on some new knowledge of the mechanism of the phenomenon -- Masaji Matsuzaki (Japan) -- There have been several reports that electrical stimulation of the brain induces the D--state Physostigmine and butyrate show an antagonistic effect on each other in the forebrain, but both produce the D--state in the lower brain stem -- Charles Fisher (U S A ) -- These revolutionary discoveries On the nature of sleep and dreaming have important implications for psychoanalytic theory The most difficult theoretical problem has been that of linking the concept of instinctual drive to this discovery of a major biological cycle involving possible alternating phases of energy build--up and energy discharge This is at the very heart of the mystery of the mental apparatus and basic motivational forces -- William Dement (U S A ) -- There is certainly no need for such a bewildering variety of terms for this state Hartmann is most uncritical in his consideration of the two "dream deprivation" experiments he cites and does not give adequate attention to the fact that there is so much more of the D--state in the newborn organism Never before in the history of biological research has so much been known about something from a descriptive point of view, with so little known at the same time about its function -- Reply: Ernest L Hartmann (U S A ) -- The multiplicity of names bears witness to the fact that there is something here worth naming Indeed, most of the names are useful, practical designations which refer to some aspect of this "state " In the term D--state: "state" emphasizes the differences from ordinary sleep as well as from waking, and "D" emphasizes the concomitant psychological state (dreaming) which in our opinion is worth keeping in mind -- The Antidepressant Drugs Anthony Hordern (England) -- This article reviews the epidemiology of depressive states, outlines the development of the antidepressant drugs, discusses problems of their evaluation, and suggests a treatment program for depressed patients Reactive depressions in young people do not require antidepressant therapy Phenelzine is useful in atypical depression with hysterical and phobic features Severe depressions require hospitalization where treatment with ECT or iminodibenzyl derivatives is usually required -- Discussions: -- Lothar B Kalinowsky (U S A ) -- Since seemingly well--planned studies show amazing differences in conclusions, my preferences are based on clinical experience rather than on statistical data ECT is the treatment of choice in severe depressions, in suicidal patients, and often because of the practical necessity of avoiding long hospitalization -- William E Bunney, Jr (U S A ) -- There are two critical areas that require comment: the complacency which develops following the successful treatment of the acute phase of what is often a chronic and malignant disease; and the data on the mechanism of action of the antidepressants which may contribute to our understanding of etiology and put treatment on a more rational basis -- Gerald L Klerman (U S A ) -- This paper has many limitations The material chosen is highly selective, the presentation is unjustifiably preferential, and conclusions based on personal opinion are not distinguished from those which have been established by investigation -- Epidemiology of Mental Retardation Ernest M Gruenherg (U S A ) This article reviews the literature on the epidemiology of mental retardation and provides a general orientation to the field Epidemiologic data can often throw light on questions which the investigator did not originally have in mind Two new approaches hold promise: the development of mental disorder registers and the national health survey -- Discussions: -- L S Penrose (England) -- The measurement of the incidence of mental retardation appears to be simple, but is in practice an extremely complicated affair Competent observers in similar communities have reported percentages ranging from 05 to 30 Such differing opinions can be attributed to inconsistencies in the aims and methods of investigators -- N Schipkowensky (Bulgaria) -- This is a wise and profound survey that unfortunately omits not only the work of the Socialist countries, but also research / u.a. Condición: Gut. S. 281-370; 8°. Geheftet. Gutes Ex.; Einband etwas berieben; geringe Gebrauchsspuren. - INHALT : Editor's Notes - Eight Ages of Man Erik Erikson (U S A ) - Eight stages of psychosocial development are distinguished, and the specific nuclear conflicts for each state are indicated The solution of each nuclear confl…ict is based on the integration of earlier ones - Discussions: - Lois Murphy (U S A ) - Erikson's concepts integrate psychodynamic thinking with awareness of the cultural matrix These concepts will be applied, revised, and developed just as the concepts of defense and autonomous functions of the ego have been absorbed and revised - Maurice Green (U S A ) - The term "crisis" is unfortunate for its connotation of almost life-or-death significance These transitional periods should be regarded more as opportunities and challenges than as crises Psychoanalysis seems to have a certain contempt for the successful, middle-class adolescent who has no serious difficulty - Jules Henry (U S A ) - Underneath the cliches, this is a psychoanalytic manual to provide personnel for an expanding economy, imperious in its job demands, determined in its conformity requirements, and savage in its punishment of those who fail - Edgar Friedenberg (U S A ) - Nobody has written of adolescence with more insight, responsibility, and real respect and tenderness than has Erikson He has never been equaled for compassion and, above all, for immediacy - Hypochondriasis: A Study of Some Historical, Clinical, and Social Aspects F E Kenyon (England) - There is no such entity as primary hypochondriasis; it always occurs as part of some other syndrome, usually a depressive one Social and cultural factors are important in influencing not only what is accepted as illness but also its form, incidence, and treatment - Discussions: - Irving K Zola (U S A ) - There seems to be something meaningful in the concept-if only demonstrated by its continued medical and lay use We also know that, when operationalized by some form of measurement, significant statistical differences are obtained between a large number of subpopulations, be they social, psychological, or diagnostic - Murray Wilson (Canada) - The concept of "hypochondriasis" should be retired from active duty in psychiatry Somatized psychic distress is too often treated by "pseudo science"! - John D Stoeckle (U S A ) - The implicit clinical phenomena are: (1) bodily complaints, (2) attitudes and beliefs about the body, (3) concerns about illness, and (4) the act of complaining to the doctor - Stewart Wolf (U S A ) - More than one hundred years of study have failed even to provide a generally serviceable definition of hypochondriasis Much of the confusion in psychiatric literature has been created by each man using a common term for his own purposes - Henri Ey (France) - Hypochondriasis is a pathological form of human existence All men of all times and all countries are preoccupied with their health, in the same measure as existence is a problem, lived in anxiety, and that anxieties attached to the body become "embodied" in the human condition - The History of Psychiatry: A Cultural and Bibliographical Survey - George Mora (U S A ) - More work is needed in critical understanding of the classic psychiatric texts, presentation of psychiatric concepts in their cultural background, and the investigation of particular cultural topics and their psychological implications - Discussions: - Edwin G Boring (U S A ) - Psychiatry could well accept the description of its portion of the psychosocial matrix as a proper goal for its historical endeavor Its selection will not, however, remain strictly bound, for knowledge is continuous and the consideration of inter-disciplinary contexts are necessary for adequate description - Oskar Diethelm (U S A ) - There is need for more knowledge of past, as well as of present, psychiatric disorders and their treatment in the hospital and in the community, in different countries and cultures - Nina Ridenour (U S A ) - "New" patterns may seem new if one is unaware of what went before Study of history might moderate the excesses of logical positivism, and the sacrifice of significance to methodology - Epidemiology of Mental Retardation Ernest M Gruenberg (U S A ) (continued from Vol 2, No 1) - Discussions: - Brian MacMahon (U S A ) and Ian Leek (England) - Gruenberg indicates that there is insufficient evidence for rejecting the "cohort" hypothesis explaining the increase in risk of mongolism with increasing maternal age Additional information is here presented that allows a fairly clear refutation - Ernest Gruenberg (U S A ) and Ari Kiev (U S A ) - Mental retardation does not confer a fixed destiny on all individuals Studies are needed to estimate the frequency of benign, self-limiting, and neglected mental retardation. Gutes Ex.; Einband etwas berieben; geringe Gebrauchsspuren. - INHALT : Editor's Notes - Psychiatric Aspects of the Prevention of Nuclear War - Group for the Advancement of Psychiatry (U SA ) - An understanding of the socio-psychiatric factors bearing on the problem of war and peace may b…e necessary if civilization is to survive - Discussions: - Arthur Lall (India) - The valuable contents of their report justify further research which could be conducted with advantage in collaboration with other behaviorial scientists - Dmitri D Venediktov (U S S R ) - The real causes of war lie in social and economic factors - Anthony J Wiener (U S A ) - The report is demonstrably incomplete, tendentious, and inconclusive - Amitai Etzioni (U S A ) - While more sophisticated than most psychiatric studies and an excellent contribution to the field, the relation between interpersonal and national processes are not sufficiently explored - Biochemical Theories of Schizophrenia Seymour Kety (U S A ) - Many of the current biochemical hypotheses in schizophrenia are original and attractive, but evidence directly implicating any one of them is hardly compelling - Discussions: - Robert G Heath (U S A ) - Kety's censure of ceruloplasmin and taraxein investigations is invalid - D W Woolley (U S A) - Kety does not appreciate the fact that new scientific discoveries come stumbling into view, not fully formed nor completely ready to defend themselves from any possible attack - Michael Shepherd (England) - The coda on genetics is the most important facet of the review - D B Losovski (U S S R ) - Biological investigations of homogenous subcategories of schizophrenia is a promising methodological approach - Henry Mcllwain (England) - While admirably objective, the review is not concerned with theories of schizophrenia and, insofar as it is concerned with biochemical theories, is curiously inverted - Jacques S Gottlieb (U S A ) - The review, critical though it is, suggests hypotheses which can only stimulate the investigator to redouble his efforts - The Independence of Neurotic Depression and Endogenous Depression - L G Kiloh and R F Garside (U K ) - A factor analysis of symptoms of depressed patients showed clustering into two groups suggesting that endogenous and neurotic depression are distinct nosological entities - Discussions: - Gerald Klerman (U S A ) - Despite methodological limitations the study is an important research endeavor - E S Averbuch (U S S R ) - Kiloh and Garside have successfully shown that endogenous and reactive depression are independent in terms of the genesis and the essence of the illness - Twin Research and Psychiatry Erik Essen-Moller, M D (Sweden) - Twin research has a unique capacity for sifting and simplifying the intricate problems of the nature-nurture controversy - Discussions: - Eiji Inouye (Japan) - Concordance rates are the starting point of research aiming at the clarification of the etiology of mental disorders; the rates themselves are not the goal of twin research in psychiatry - Martin L Pilot (U S A ) - Application of concepts closer to psychoanalytic theory should increase the value of twin studies - David Rosenthal (U S A ) - Past twin studies of schizophrenia have overestimated the genetic factor - Letters to The Editor - Recent Books. Gutes Ex.; Einband etwas berieben; geringe Gebrauchsspuren. - INHALT : Editor's Notes -- Conception of Schizophrenia Within the Last Fifty Years and Today -- Manfred Bleuler (Switzerland) -- There is no specific cause of schizophrenia; both somatic and psychological factors are involved…A schizophrenic inner life lies beneath the surface of healthy psychic life Since schizophrenia can be understood as a faulty development of the personality, the goal of therapy is to influ-ence a healthy development of ego and a healthy adaptation to reality -- Discussions: -- Jack Ewalt (U S A ) -- A scholarly summation of the current status of conceptions on schizophrenia, this article is a significant addition to the literature on this difficult and puzzling disorder -- K Leonhard (German Democratic Republic) -- There is not one, but several-or perhaps even many-schizophrenias Lack of progress in our knowledge is because of the many attempts to treat the schizophrenias as a single unified disease -- Eliot Slater (England) -- How can Bleuler be so sure that schizophrenia is an abstraction without validity merely because a few decades have not brought us the answers we want? His argument is founded on faulty logic and self-contradiction -- Frank Fish (England) -- Bleuler's assumption that there is a similarity between normal and schizophrenic experience is questionable, nor can I agree with Bleuler's concept that schizophrenia is a faulty development of personality -- Hans Hoff (Austria) -- It is true that the genesis of schizophrenia is multifactorial, as Bleuler describes it However, among the various states that are called "schizophrenic," there is a nuclear group which has a distinct inborn error of metabolism -- The Study of Folk Psychiatry Ari Kiev (U S A ) -- In many cultures, native practitioners are treating emotionally disturbed persons with as high a rate of success as our more sophisticated techniques In all cultures, customs set up the conditions under which the patient accepts without fear the authority and power of the healer -- Discussions: -- Michael Maccoby (Mexico) -- It would be a mistake to equate these primitive therapies with -- psychoanalysis, where the patient's conflicts are made conscious and he is expected to suffer the anxiety in making a decision and -- standing by it -- Jules H Masserman (U S A ) -- Psychiatrists have difficulty in accepting the fact that the world does not revolve around the individual Much is determined by circumstances and customs the individual alone does not control All of us are trained within our society with its local mores, and if we depart radically from them either in what we do or "advise," we would be impeached -- Community Psychiatry: The Newest Therapeutic Bandwagon -- H Warren Dunham (U S A ) -- There are many diverse and inconsistent concepts of community psychiatry They range from the idea of bringing psychiatric treatment to the poor, to educating nearly everyone in the principles of mental hygiene Community psychiatry, in its focus on noncritical cases, is likely to further remove the psychiatrist from bona fide cases of mental illness Discussions: Serge Lebovici (France) -- Community psychiatry aims at assuring continuity of care, in the framework of reasonable planning Awareness of the importance of ecological factors does not force one to neglect endopsychic -- conflicts -- Leonard J Duhl (U S A ) -- It is a rather outmoded question to ask whether the psychiatrist steps out of his appropriate role when he concerns himself with the whole community system Community psychiatry is a logical development of psychiatric theory and practice -- J A C de Kock van Leeuwen (Netherlands) -- It is incorrect to conclude, from its unsolved problems, that community psychiatry is dangerous; and the analogy that, as psychiatrists do not know what they do with patients, they certainly will not know what they do when they treat the community, is unjustified -- John R Seeley (U S A ) -- Everyone is entitled to see danger where others see hope But, since the expression of either is a strategy to change outcome, weighty warnings should receive careful examination -- Eric Trist (England) -- There is an enduring place in community psychiatry for two general positions, those of enlightened as distinct from unenlightened conservatism, and of pensive as distinct from ill-considered radicalism -- Louis Miller (Israel) -- Dunham contends that the psychiatrist should not "treat" larger groups in the community Although there is no need for the psychiatrist to be directly involved in community organization, he must be concerned with the social causes of dysfunction and understand the technique of mobilizing community services -- Reply: H Warren Dunham (U S A ) -- I deplore with Dr de Kock van Leeuwen that my views should be taken as a "halt" signal on the promotion of mental health Duhl merely restates, with a somewhat tired metaphor, an organic view of society Lebovici visualizes a more circumscribed role for community psychiatry than most others Seeley's accusation . // u.a.m. Sprache: Deutsch Gewicht in Gramm: 550.
