Sinopsis

Pacemaker malfunction due to electromagnetic interference and other causes is an important potential cause of patient morbidity and mortality. While the pacemaker industry has been aware of this situation, little has been communicated to physicians concerning the problem. Now, the collected experience and thoughts of pacemaker industry representatives, physicians working with pacemaker patients and legal authorities are brought together in this book to focus on the topic of pacemaker malfunction in perioperative settings. The book begins with discussion of the design and function of modern pacemakers and related devices. It moves on to consider how electromechanical interference, radio-frequency transmission, nuclear magnetic resonance imaging, radiotherapy and extracorporeal shock wave lithotripsy can interfere with or alter pacemaker function. Then, the legal perspective concerning industry and physician responsibilities to patients with pacemakers is presented, followed by discussion of indications for temporary perioperative or permanent pacing. In the final two chapters, the editors make recommendations for the perioperative management of patients with pacemakers.

A symposium, "Perioperative Management of Pacemaker Patients", was held in May 1990 at the Karl-Franzens-Universitat, Landeskrankenhaus Graz, Austria. The purpose of the symposium, organized by the Departments of Anesthesiology and Surgery, was to discuss the current status of permanent pacemaker technology, emerging developments in this field, legal issues, indications and methods for tempor­ ary perioperative pacing, and the potential for pacemaker malfunction and adverse patient-pacemaker interactions in perioperative and other medical settings. It was hoped that participants would come to some consensus concerning recommendations for satisfactory perioperative management of pacemaker patients. This symposium was probably the fIrst occasion ever that brought together representatives of the pacemaker industry, implanting physicians, and physicians responsible for the management of pacemaker patients in perioperative and other hospital settings to discuss these important matters. Certainly, it was recognized by all who attended that there existed at the time little in the way of substantial knowledge of how to best manage pacemaker patients or patients with possible indications for temporary pacing in "unique" hospital settings especially with exposure to electroma­ gnetic and other potential interference, but in the absence of professional persons with direct knowledge of pacemakers and related devices. Much new information was presented and useful ideas exchanged, so that three symposium participants were of the opinion that papers presented at this meeting should be published as a collective work.

The book was first conceived at a symposium held at the University of Graz, Austria in May 1990: Perioperative Management of Pacemaker Patients. Symposium participants included representatives of the pacemaker industry, implanting surgeons, cardiologists, anesthesiologists, radiologists and other physicians involved in diagnostic and therapeutic interventions in pacemaker patients. There is a wealth of new information presented in this book, addressing the issues of diagnosis, prevention, and management of pacemaker malfunction in hospital settings. Further, the editors' approach to the problem is novel in that they have recruited authorities from a number of disciplines to present their views.
