NOTARIZING A DEFAULT ON A PROMISSORY NOTE, in a partly printed document, completed in manuscript, April 17, 1818, in Cleveland, Ohio, and signed by A.W. Walworth, as notary public attesting to its provisions: James Hezless promising to pay "Day & Swift" 500 dollars at the Commercial Bank of Lake Erie within 90 days.; Those provisions are given in the upper half of the document (headed "United States of America" which is printed around a patriotic cut of the American eagle, 2 ¼ x 1 ¾ inches), with the default printed in the lower half (appropriate names entered in manuscript): when presented with the note as 90 days came due, it could not be cashed because Hezless had not deposited the money at the bank
