Daher sollten Patienten, bei denen das Risiko einer Wiederaufnahme besteht, frühzeitig während ihres Krankenhausaufenthaltes identifiziert werden, um rechtzeitige Interventionen und eine angemessene Entlassungsplanung zu ermà glichen. Dies kann durch den Einsatz prädiktiver Analysen erreicht werden, die anhand der Daten vergangener Patienten und deren Krankenhausaufenthalte das Risiko einer ungeplanten Wiedereinlieferung für neue Patienten prognostizieren. Während die bisherige Forschung bereits vielversprechende Ergebnisse zum Einsatz solcher Analysen zur Erkennung von Hochrisikopatienten für vereinzelte Diagnosegruppen zeigt, werden der Einsatz dieser Modelle in der Praxis sowie die nachfolgende Verwertung der Ergebnisse bisher vernachlässigt. Um diese Forschungslücke zu schlieÃen, besteht das Hauptziel der Studie in der Unterstützung der Entscheidungsfindung im Entlassungsprozess in Krankenhäusern durch die Entwicklung verfahrensspezifischer Vorhersagemodelle zur Identifizierung von Hochrisikopatienten ungeplanter Wiedereinlieferungen. Im Rahmen einer Case Study in Zusammenarbeit mit einer australischen Krankenhausgruppe werden Patienten und Episodendaten von fünf Jahren aufbereitet und analysiert. Auf der Basis bestehender Literatur sowie des vorliegenden Datensatzes werden spezifische Einflussfaktoren auf das Risiko einer ungeplanten Wiedereinlieferung für die sechs wichtigsten Prozeduren im australischen Gesundheitssystem erarbeitet. Darauf aufbauend werden verschiedene Machine Learning Methoden zur Entwicklung prädiktiver Modelle für jede Prozedurengruppe individuell getestet und evaluiert. Die resultierenden Modelle werden in ein intelligentes Entscheidungsunterstützungssystem integriert und kà nnen dort auf neue Patientendaten angewandt werden, um Handlungsempfehlungen für das Krankenhauspersonal im Entlassungsprozess zu entwickeln. Der entwickelte Prototyp wird abschlieÃend im Rahmen einer Umfrage mit medizinischem Fachpersonal auf seine Effektivität, Nützlichkeit und Akzeptanz getestet. Die Ergebnisse der Studie bestätigen die Eignung des Systems zur frühzeitigen Erkennung von Risikopatienten für eine Verbesserung der Ressourcenverteilung sowie des Entlassungsmanagements im Krankenhaus. Die Erkenntnisse der Studie liefern wichtige Impulse für die Eignung der Implementierung von Machine Learning-Methoden in Forschung und Praxis. Eine verbesserte Unterstützung im Entscheidungsprozess der Patientenentlassung kann dazu beitragen, potenzielle Wiedereinlieferungen zu verhindern und dadurch die Krankenhauskosten zu senken sowie die Qualität der Gesundheitsversorgung zu erhà hen. Mithilfe der Vorhersage der voraussichtlichen Kosten und des Wiederaufnahmerisikos kann das Krankenhauspersonal fundierte und kosteneffizientere Entscheidungen über die Entlassung von Patienten treffen und diese strukturierte und systematisch umsetzen.
