In order to understand the background of this technique, it is necessary to compare the physical p…roperties of blood, which reflects ultrasound poorly but moves with high velocity (up to 150 cm/s) with those of the myocar dium, which reflects ultrasound strongly but moves with low velocity (less than 10 cm/s). In tissue Doppler imaging, existing Doppler technology has been modified to bypass the high-pass filter and enhance calculation of low velocities, thus enabling selective visualization of the myocardium rather than of the blood. Because the color Doppler tissue images are super imposed on the conventional two-dimensional ultrasound images, this technique is known as TDE. Following a brief introduction, the history of ultrasound and Doppler imaging is presented. It is now about 150 years since the death of Christian Doppler, who described the 'Doppler' effect, and more than 100 years since Pierre Curie discovered the piezoelectric effects of crystals. TDE was developed by Nobuo Yamazaki and Yoshitaka Mine at the Medi cal Engineering Laboratory, Toshiba Corporation, Tochigi, Japan. In recent years, our knowled…ge of the earliest stages of arteriosclerosis have greatly improved. By now, we have learned to relate morphologic changes to disturbances in function. It has been of particular impor tance that components of the arterial wall could be analyzed in regard to dysfunction, for example, in the endothelium or the vascular smooth muscle. The interaction of the different morphological components of the vascular wall could thus be much bet ter understood. Likewise, the interaction between the arterial wall and the flowing blood could be much better described, including the intimate relationship between platelets and the endothelium, the coagulation system and the endothelium, the granulocytes and the endothelial cell layer, as well as processes of migration of blood cells into the subendothelial space. The recognition of functional and morphological disturbance has attained clinical significance not only because the arteriosclerotic diseases have quantitatively reached the dimensions of an epidemic, that is, of a magnitude never been witnessed. In end-stage heart disease, procedures are now performed routinely which were only experimental one or two decades ago. Heart transplantation has become… a well-established procedure. According to the Gen eral Registry of the International Society of Heart and Lung Transplantation. 40,738 heart transplantations had been performed through the end of 1997 with survival rates of 78 % at one year, 65 % at five years, and 42 % at ten years. The progress in this field has been due to intense efforts in understanding and modulating immune responses to the trans planted heart, to elaborate therapeutic strategies to constrain infections, and to improved out-patient care. Accordingly, heart transplantation is integrated into the facilities of the health care systems, the routine of physicians, and the awareness of patients. The resulting increase in potential organ recipients has not been met, however, by an equivalent increase of available donor organs. This increasing discrepancy has forced the technical improvement and clinical evalua tion of mechanical circulatory support systems as an option for treatment of critically ill patients with a failing heart. Initially, these assist devices were only used to maintain sufficient circulation in post-cardiotomy heart failure until myocardial function had recov ered. This development of the discovery of a peptide hormone is unparalleled in the sense that it was mostly morphologists who contributed to the final characterization of these cardiac hormones. Physiology, pharmacology, and molecular biology have… uncovered a wealth of information on structure and function of this heretofore largely neglected 'organ'. Clinical medicine is now called upon to define the clinical significance of these observa tions that imply the mechanisms of blood coagulation, e.g., the interaction of throm bocytes with the endothelium, vasomotor control, and specifically, the regulation of smooth muscle tone with consequences for vascular resistance and conductance and organ blood flow. Finally, metabolism of lipids with the everlasting problem of athero sclerosis is an important aspect. In a second step, implications regarding the improvement of current therapeutic con cepts, as well as the development of new modalities of pharmacotherapy will have to be discussed. The topic addressed by the 1990 Gargellen Conference: Endothelial Mechanisms of Vasomotor Control, clearly is of interest for both basic scientists and clinicians. It has been the aim of the organizers, the Society for Cooperation in Medical Science (SCMS) with this and the previous symposia to foster and support both basic science and clinical research. Research in medicine today shows two major directions of development: on the one hand, increasing involvement of the basic sciences and their methodology. On the other hand, statistical validation of concepts and therapeutic strategies in large scale population-and multicenter-studies.

Thereby, the knowledge about physiology, pathophysiology, and clinical circumstances of cardiocirculatory and re spiratory regulation during exercise were significantly extended. The working groups of W. Hollmann, Cologne, and K. Wasserman, Los Angeles, determined normal values for the gas-exchange parameters and derived values for healthy normals in large populations. Wasserman and coworkers were able to introduce a differential diagnostic concept for patients suffering from various cardiovascular and cardio pulmonary diseases. Many cardiologists, working, for example in myocardial failure or with rate-adaptive pacemakers, belong to those who recommended the modem, computerized ergo spirometry. Die Bilder des Hydrops foetus universalis, des Icterus gravis und auch der Neugeborenen-Anämie sind sehr lange bekannt. Aber erst v. GIERKE (1921) brachte sie auf einen Nenner, indem er das Gemeinsame mancher Veränderungen heraushob, ohne allerdings eine brauchbare Erklärung für die Pathogenese dieser sich so verschieden darbietenden Erscheinungen bringen zu können. Erst die neuen Entdeckungen, insbesondere amerikanischer Forscher, auf dem Gebiet der Blutgruppen brachten plötzlich Licht in das unklare Geschehen, für das so viele Theorien aufgestellt waren. Nachdem nuh die engen Zusammenhänge zwischen dem eigenartigen Verhal ten des Blutes der Eltern und der Erkrankung der Frucht einwandfrei festgelegt waren, zeigte sich überraschend die ungeheuere praktische Bedeutung und die zahlenmäßige Ausdehnung des ganzen Problems, aber auch manche Schwierigkeit in der Beweisführung. Die bisherigen Sitzungen zeichneten sich durch aktuelle The men mit kompetenten Referenten aus, so daB sie immer ein ganz auBerordentliches Interesse nicht nur bei den Kardiotechnikern, sondern auch bei den tibrigen Tagungs teilnehmern fanden. Infolge dieses groBen Echos haben wir uns entschlossen, die Beitrage 1990 zu sammeln und mit Hilfe der Referenten zu veroffentlichen. Das Titelblatt reprasentiert das Plakat zur 19. Nach einer Ubersicht tiber die Auswirkungen des Einsatzes der Herz-Lungen-Ma schine mit Perfusat und Hypothermie auf den Organismus, betrachtet unter physiologi schen Aspekten, wird ein uns heute besser bekannter Funktionskreis mit dem humora len System und Mediatoren angesprochen. Erganzt wurde das Kapitel zur extrakorporalen Zirkulation im Erwachsenenalter. Ein weiterer Beitrag ist der EKZ bei Neugeborenen und Sauglingen vorbehalten und gibt die wesentlichen Details fUr diese schwierige Altersgruppe wieder. Die unter Ganzkorper-Hypothermie-Bedingungen eingeleitete und durchgefUhrte Myokardpro tektion mittels Kardioplegischer Losung nach Bretschneider zeigt einige Ergebnisse der sogenannten 'high volume cardioplegia' auf. Eine freimutige Diskussion ermog lichte es, 'viele' auszufragen, aber auch 'nein' zu sagen bei fehlendem Einverstand nis. Zum Einverstandensein mit Falschem wurde niemand gedrangt. DaB in vielen Fragen - noch kein Einverstandnis erreicht werden konnte, lag sicher nicht am Nichtfragen begrundet, sondern in der mangelnden Kenntnis uber Auslosung und Entwicklung der Krankheitsphanomene.
