This has been the result of increased recognition by psychologists of the potential role they can play in evaluating patients involved in lawsuits and the increasing sophistication of lawyers who have recognized that damages can go beyond claims of physical and motoric impairment. However, this increase in involvement has not always been matched by a more sophisticated recognition of how the client (whether plaintiff or defendant) can best be served by the neuropsychologist. I have personally seen or reviewed many cases in which the psychologists involved did not effectively present their case. This partially occurred because they failed to recognize the difference in presentations aimed at clinical audiences and those aimed at a legal proceeding. It also occurred because they failed to recognize that the standard of proof necessary is very different in a hospital and in a courtroom. Finally, it occurred because they rarely recognized what the unique role of the psychologist can be in either countering or supporting the testimony of medical specialists. The purpose of this volume is to bring some light on these questions. The presentations here are write-ups of the work presented at the Third Annual Conference on the Luria-Nebraska Neuropsychological Battery held in Omaha during May, 1985. All of the papers emphasize the role of the Luria-Nebraska but the ideas can be used with almost any test.

Microscopic examination indicates that the 'white' matter regions are characterized by a multilamellar sheath (myelin) which sur rounds the axons. A morphologically similar myelin imparts the white color to tracts of the peripheral nervous system, although, as is empha sized throughout the book, there are very significant morphological and chemical differences between central and peripheral myelin. The rapid postnatal accumulation of myelin is temporally associated with increasing functional capacity and presumably indicates some important role related to neuronal activity. The ability of myelinated axons to conduct electrical impulses much more rapidly and with much lower use of energy, relative to unmyelinated axons, is now generally accepted. As is evident from the above capsule description, a considerable body of information concerning myelin is available. However, as in other areas of research the unknown expands before us more rapidly than we are able to add to our core of knowledge. This is a corollary of the axiom that a good experiment raises more questions than it answers. Among the areas currendy under investigation in many laboratories are the following: interaction of myelin-forming cells with neurons, assembly and metabo lism of myelin, molecular architecture of myelin, and the details of the functional role of myelin.

In our western civilization, the bulk of attention has focused on those indi viduals who specifically seek treatment or those who have become so disabled by these problems that they require treatment. These indi viduals usually qualify for a psychiatric diagnosis of alcohol or other substance abuse. However, just as it has been recognized that primary substance abuse is frequently associated with other diagnosable psychi atric disorders, such as sociopathy or attention deficit disorder (residual type) and that the origins of substance abuse are multivariate, we have also begun to become aware that many other individuals in our society with psychiatric or other problems also suffer, to varying degrees, from substance abuse. These problems may be considered secondary by vari ous specialists or treatment personnel; but nevertheless, they are prob lems, and what disorder is primary or secondary in a given individual may often be very difficult to determine in a meaningful fashion. Thus, within the past decade, research studies have reported significant inci dences of substance abuse/or misuse in high school and college-aged populations, in medical populations, and in individuals with other psy chiatric disorders such as schizophrenia, depression, and the anxiety and personality disorders. Yet to date little has been done to bring together and systematize this widely scattered data that describes the presence of substance abuse problems in various populations.
