With increasing numbers of sonography systems entering the ambulatory office setting, obstetric sonography at a routine level (level I) has largely been the targeted area. Recent developments in gynecologic real-time sonography have, however, significantly enlarged the sphere of applicability of sonographic equipment in an office setting. The very rapid growth of follicular sonography in infertility assessment and management has made real-time sonography of increasing importance to the gynecologic practitioner. In office settings like the authors', gynecologic office sonography represents close to 50% of all ordered sonography. This handbook of office sonography in obstetrics and gynecology was conceived to reflect these changes in practice patterns. This volume is not meant to replace standard sonography texts for the full-time sonographer but is instead directed toward the practicing obstetrician/gynecologist who uses real-time sonography in the office setting within the framework of daily practice. Technical comments were therefore restricted to a minimum, with practical advice and photographic examples taking their place. Most of the sonographic real-time images were retrieved from the authors' own files. However, some were obtained through the generosity of friends and colleagues, for which we would like to extend acknowledgment and appreciation. This is because, until a few years ago, no real in-depth knowl edge of computers and computerized equipment was at the disposal of those with a nonscience, nonquantitative background. Some people from other disciplines-including business, the arts, and the social sciences-had been working with such equipment, but they had 'gone over' and tended to be even more computer-conscious than those whom they served. It is only comparatively recently that people like Elisabeth Gerver with a firm arts and adult education background first of all became knowledgeable and then remained true to that background. To her eternal credit, Elisabeth Gerver, when she became involved, avoided being sucked into the world of the jargon or even that of the thinking of the computer scientists and the electronic engineers. On the contrary, she insisted that she was an educated woman dealing with other educated people, and that they would all speak in the language of everyday discourse. It worked! One consequence of her experience and her thought is this remarkably lucid and readable book. It will prove to be of immense value to many in the world of adult and community education. But the beneficiaries will run beyond those sectors of society. The impetus for the study was an article in the Los Angeles Times about architectural interventions that were planned for several noise-impacted schools under the air corridor of Los Angeles Interna tional Airport. These interventions created an opportunity to study the same chil dren during noise exposure and then later after the exposure had been attenuated. The study was designed to test the generality of several noise effects that had been well established in laboratory experimental studies. It focused on three areas: the relationship between noise and personal control, noise and attention, and noise and cardiovascular response. That is not to say that all persons with mental retardation experience behavior disorders; how ever, most studies indicate that the incidence of emotional disturbance in this population is four to six times greater than that observed in similar intellectually nonhandicapped children and adults. It is equally well known that the principal form of treatment accorded clients with mental retardation and behavior disorders is pharmacotherapy or the prescrip tion of behavior modifying drugs. Recent studies show that 6 out of every 10 individuals with mental retardation have been prescribed drugs as treatment for disorders of emotion or behavior. Unfortunately, further studies indicate that only one or 2 out of every 10 clients receiving medication are determined to be 'responders,' such that some thera peutic benefit is derived from their drug treatment. As noted by the title, the single major thrust of this volume is to review approaches to the treatment of behavior disorders in persons with mental retardation from a nondrug perspective.
