Major advances in folate research made within the last decade include (i) the approval of pemetrexed for the treatment of lung cancer and mesothelioma, and (ii) the demonstration that cell membrane-anchored folate receptors (FR) are exploitable for cancer and inflammatory disease management. FRs are not widely distributed in normal tissues, except on some luminal surfaces; however, they are accessible to systemically administered agents when expressed on many cancers as well as on activated macrophages involved in various inflammatory diseases. High affinity folate-radioisotope conjugates have been developed for imaging pathogenic FR-positive diseases, including cancer. Since the FR transports folates via a low capacity but high affinity endocytic pathway, a variety of FR-targeted antifolate drugs and folate conjugates bearing a wide range of payloads (including cytotoxic drugs) are currently being developed which exploit this property. Furthermore, for most solid tumours there is a pre-invasive or intraepithelial stage of disease. This affords the opportunity for early detection and prevention of invasive disease and hence a cure. However, with this advancing knowledge comes a whole plethora of questions which will be explored in this monograph.Firstly, we need to understand the global burden of pre-invasive disease and what the public health implications might be for wide-scale screening programmes. In the western world we already have experience of screening for cervical, breast, prostate and more recently colon cancer. As well as their potential benefits these programmes have financial and psychosocial implications which need to be carefully weighed. This is especially true since many pre-invasive lesions will not progress to cancer in a individual¿s lifetime. In addition, there are questions concerning whether screening reduces the cancer burden or in fact distorts the survival figures through lead-time bias.Secondly, at the level of epidemiology and molecular pathogenesis there are important questions regarding the aetiology of pre-invasive lesions; an understanding of which might lead to possible chemopreventive strategies. Their purpose was to focus on advanced technologies for gen…erating, radiating, and detecting UWB,SP signals, on mathematical methods, their propagation and scattering, and on current as well as potential future applications. The success of these two conferences led to the desirability of scheduling a third conference. Impetus was provided by the electromagnetics community and discussions led by Carl Baum and Larry Carin resulted in the suggestion that the UWB conferences be moved around, say to government laboratories such as Phillips Laboratory. Consequently the decision was made by the Permanent HPEM Committee to expand AMEREM '96 to include the Third Ultra-Wide Band, Short-Pulse (UWB,SP 3) with the Third Unexploded Ordnance Detec tion and Range Remediation Conference (UXO) and the HPEMINEM Conference in Albuquerque, New Mexico during the period May 27-31, 1996. Planning is now underway for EUROEM '98 in June, 1998 in Tel Aviv, Israel. Joseph Shiloh is the conference chairman. A fourth UWB,SP meeting is planned as a part of this conference and Ehud Heyman will coordinate this part of the meeting.
