Paperback. Condición: New. In January 1937, Nobel laureate in Physics Subrahmanyan Chandrasekhar was recruited to the University of Chicago. He was to remain there for his entire career, becoming Morton D. Hull Distinguished Service Professor of Theoretical Astrophysics in 1952 and attaining emeritus status in 1985. This is wher…e his then student Ed Spiegel met him during the summer of 1954, attended his lectures on turbulence and jotted down the notes in hand. His lectures had a twofold purpose: they not only provided a very elementary introduction to some aspects of the subject for novices, they also allowed Chandra to organize his thoughts in preparation to formulating his attack on the statistical problem of homogeneous turbulence. After each lecture Ed Spiegel transcribed the notes and filled in the details of the derivations that Chandrasekhar had not included, trying to preserve the spirit of his presentation and even adding some of his side remarks. The lectures were rather impromptu and the notes as presented here are as they were set down originally in 1954. Now they are being made generally available for Chandrasekhar's centennial.

GER This compact guide aims to define the current approach to hypertension in practice, with the focus on the individual whom the physician faces across his desk. In the population, blood pressures are distributed along a Gaussian type of curve, but with some tailing towards the upp…er range. A systolic blood pressure of 160 mmHg is commonly accepted as the upper limit of the normal range. For diastolic pressure, the gradings are much more detailed: borderline hypertension (90-94 mmHg, mild hypertension (95-104 mmHg), moderate hypertension (105-114 mmHg), and severe hypertension (115 mmHg and over). Despite its skewing to the right, the bell shape of the distribution curve of blood pressures implies that the milder elevations of blood pressure are the most common. Such pressures are observed in 15-20% of the population upon casual measurement. After rechecking - which is mandator- the prevalence of hypertension drops to some 5%, due to psychological and statistical factors. Even this modest segment of the population represents an important proportion in terms of future cardiovascular risk. It is an essential part of preventive and curative health care to identify these people; the more so, because some 40% of the excess risk has already been proven to be reversible by conventional antihypertensive treatment. I remember the surgeon who, when I was a student, returned a referral saying he would operate on the man when his dementia got better. Feeling superior, and encouraged by the consultant psychiatrist, we students laughed a good deal at this. Before we finished clerking on that Unit a visiting Professor of Psychiatry had demonstrated the reversibility of the symptoms of dementia in a patient with a rare metabolic disorder. Perhaps ignorance is sometimes an advance on received wisdom. The lesson is the concept of dementia must always reflect the state of knowledge and is therefore in a sense ad hoc. Secondly, what the criteria for, and also who the arbiters of, the diagnosis might be is not always clear. The withdrawn patient who avoids social contacts and obligations and is often slowed up in mind and body. Differential diagnoses in clude schizophrenia, CVA or tumour, hysteria and semi-coma includ ing drug intoxication. Withdrawn and retarded patients with depres sive illness are at risk of failing to eat or care for themselves. C. The suicidal patient. May present as unexpected, inexplicable coma; a badly cut patient may be confused by the doctor with acci dent or assault. The immediate emergency is medical or surgical: treatment is for coma, bleeding or asphyxia, and requires immediate admission to casualty. The first presentation of depression is always a minor emergency as it may be the only attempt the patient makes to see a doctor. Diagnosis must be positive, based on the recognition of depres sive features, not negative, based on the exclusion of other dis eases. The cardinal symptoms of depressive illness: 1. I would agree with Carson Dick that aspirin and phenylbutazone are way down the list of drug priorities following the marketing of other NSAIDs that are effective and safer. A similar change of attitude to the management of chronic bronchitis brought many new patients to the chest clinics in place of the vanishing tuberculous population. The writers derive their own experi ence from a range of cases less highly selec…ted than those on which textbooks are traditionally based. They are also in a good position to pick out topics which they see creating difficulties for the practitioners of their district, whose personal capacities are familiar to them; and to concentrate on contexts where mistakes are most likely to occur. They are all well-accustomed to working in consultation. All the authors write from hospital experience and from the viewpoint of their specialty. There are, therefore, matters important to family practice which should be sought not within this series, but elsewhere. Within the series much practical and useful advice is to be found with which the general practitioner can compare his existing performance and build in new ideas and improved techniques. Absolute diagnostic accuracy may not be possible, particularly in the field of primary care, when management has to be on analyis of symptoms and on knowledge of the individual pa…tient and family. This series follows that on Problems in Practice which was concerned more with diagnosis in the widest sense and this series deals more defini tively with general care and specific treatment of symptoms and diseases. Good management must include knowledge of the nature, course and outcome of the conditions, as well as prominent clinical features and as sessment and investigations, but the emphasis is on what to do best for the patient. Family medical practitioners have particular difficulties and advantages in their work. Because they often work in professional islation in the com munity and deal with relatively small numbers of near-normal patients their experience with the more serious and more rare conditions is restricted. They find it difficult to remain up-to-date with medical advances and even more difficult to decide on the suitability and application of new and rela tively untried methods compared with those that are 'old' and well proven. Their advantages are that because of long-term continuous care for their patients they have come to know them and their families well and are able to become familiar with the more common and less serious diseases of their communities. It assumes that the neonate comes into the world with the endowment and the need to contact others, to attach itself to the surrounding humans and by this very endowment enforces these humans to attach themse…lves to it. The book follows the main vicissitudes in this process and describes in short outlines some of the disturbing manifestations of failure at different ages in the smooth progress of socialization - the process by which the child adapts to the requirements of the social group of which he is a member. In accordance with recent studies the book stresses the influence on social development of the wider environment. It is no longer believed that the quality of the early mother child relationship exclusively determines all later develop ment. The book also surveys the present state of assessment and treatment methods and tries to indicate where short intervention by the developmental paediatrician might be sufficient and where more long-term intensive treatment by specialists would be needed. The author agrees with A. M. and A. D. B. Clarke (1976) that early experiences, though powerful, are not irreversible, and that with changes in environmental circumstances, and with under- 9 Social development standing of the main causal factors for the deviance, the effect of adverse early events may be modified and develop ment towards normal social progress may to some extent be restored. The rates of maternal and infant morbidity and death for these countries are extremely high but much of the morbidity and death is preventable, even with the limited resources available for health care in many parts of the world. The resources devoted to maternal and child care should be greatly expanded, but even the most hopeful projections will leave a wide gap between human needs and available services. WHO estimates that two billion deliveries in the remaining two decades of this century will not be attended by a trained person. Paperback. Condición: New. This book contains a consensus on menopause research, a consensus reached during the First International Congress on the Menopause in June, 1976. The Congress brought together about 165 people, most of whom are engaged in research in this field, in the resort town of La Grande Motte, near Montpellier,…France. It was planned so that the main emphasis would be on the exchange of information in small Workshop sessions. Workshop Chairmen were asked to present summaries of their deliberations at the closing session. It is these summaries that form the essence of this text. The Congress also included six Free Communications sessions. They are represented in this book by a small number of papers printed in full (selected from some 40 contributions in all) that in the opinion of the editors, contain data that particularly complement the Workshop summaries. All other papers are mentioned in these Pro ceedings by titles and by the name, affiliation and address of the first author. Organizing this Congress was a stimulating and heart-warming experience. It was done by the staff of the International Health Foundation in Brussels and Geneva, under the considerate auspices of the American Geriatric Society and the Medical Faculty of the University of Montpellier.
