As a result, the doctor began leading a double life: a well respected surgeon (then working at Manhattan's Mount Sinai Hospital) and socialite by day, and Il Dottore, a gambling and sex addict by night. As a result of his relationship to the mafia, the doctor was welcomed into an exciting and often glamorous underworld, which included disco, drugs, high~stakes gambling, and, of course, beautiful women. Yet somehow, despite his nightly diversions, Il Dottore continued to pursue his profession seriously, and in time, became one of the nation's leading cardiac surgeons. Ultimately, the high life came crashing down for everyone involved when the heads of the five New York Mafia families were imprisoned after the Commission trial~led by then~Attorney General, Rudolph Giuliani~in the mid~1980s. The pivotal moment in the doctor's life and career came when the government's star witness, a man named Ralph Scopo, lay on his operating table. The doctor was to perform bypass heart surgery on Scope, but, of course, it wasn't that simple. In one ear, he received a directive from John Cotti, whose words were, "When it's over, make sure that only one of you comes out of the operating room breathing." In the other ear, Giuliani and his men threatened to ruin the doctor's career if Scopo were to die on the operating table. The doctor was thus torn between his loyalty to La Cosa Nostra and his devotion to the Hippocratic Oath. His decision changed his life forever.
