I wish to thank Dr. William A. White, Superintendent of St. Elizabeth's Hospital, for his permission and support in making this experiment, Dr. Nolan, D. C. Lewis and Dr. Gertrude H. Davies for their assistance in studying the cases, and Dr. Shepherd Ivory Franz for his advice and criticism in preparing this thesis. Dr. Lewis furnished the reports of the physical examinations which are included in the case histories.
